MEDICPASS is designed to give you a head start on the blood donation process by allowing you to answer many of our blood donation screening questions on your own device before you come in to donate.

PLEASE NOTE: Your MEDICPASS questions MUST BE STARTED AND COMPLETED ON THE DAY OF YOUR DONATION. MEDICPASS will not tell you whether or not you are eligible to donate blood (this will be determined when you answer the remaining questions at your donation site). Existing donors can check their eligibility dates in the donor portal or the app.

I have a valid picture ID. ID is mandatory for new/first-time donors.
I understand that my eligibility to donate will be determined after I present to a collection location and finish screening.
I am completing this pre-donation questionnaire on the day I plan to donate.
I am in a private setting and will answer the donation questions without assistance from anyone else.
I consent to receive a text with my MEDICPASS code. MEDIC will never sell your personal information to any third party.

COMPLETE YOUR MEDICAL HISTORY

It's important that you answer all questions in a private, confidential setting and that you read each question in its entirety before answering, including linked lists or information. Feel free to skip any question if you are unsure how to answer.

MEDICPASS QR CODE

A link to your QR code can be texted to you, saved to your device, or printed. You must have this code to register for your donation. MEDIC staff cannot retrieve the code for you.
